Boarding Agreement Form

Thank you for your interest in our clinic for your pet’s needs. Please complete our Boarding Agreement Form.

All pets must be up to date on the following vaccines/tests:

 

Canine: Rabies, Distemper Combo, Bordetella, Canine Influenza, Fecal Exam, and we also recommend a heartworm test every year.

 

Feline: Rabies and Distemper Combo

 

Pets must be fleas, tick, and parasite FREE.

 

If we see evidence of fleas, ticks, or intestinal parasites, we will treat your pet accordingly without contacting you.

 

You may bring toys, bedding, blankets, etc. with your pet. We will make every effort to return them in good condition, but we cannot guarantee that they will go home in the same condition they arrived in. We can provide any bedding or food for your pet, with the exception of some prescription diets.

 

We will ask you for emergency contact information during each stay.

 

If your pet exhibits unacceptable aggressive behavior while boarding, we will contact you. We cannot endanger our employees.

 

One of the advantages of boarding your pet at our hospital is that a veterinarian is readily available should your pet become sick or is injured, we need to know how to handle any situation.
